A Combat Trainer Deathclaw That Got Left on the Cutting Room Floor

According to one of the game's designers, one of the intelligent deathclaws in Vault 13 was originally meant to double as an unarmed combat trainer who could teach the player new fighting skills. The idea got dropped late in development, so in the final game the Vault 13 deathclaws are a lot less chatty and useful than they were planned to be.

Two More Happy Endings That Didn't Survive

Alongside the deathclaws, both Vault City and the ghoul town of Gecko had good endings planned that never made it into the shipped game. Whatever outcome you get for those locations in the final release is, for better or worse, the only one that made the cut.

A Diner Full of the Player Characters Who Never Were

Stumble into the special encounter called the Café of Broken Dreams and you'll find a little diner, modelled on a bar from the first Fallout, populated by every Vault Dweller character model that didn't make it into the original game, including ones you were never actually allowed to play as. Characters from the first Fallout turn up too, and you can even recruit the original game's dog, Dogmeat, right there at the counter.

San Francisco's Fuel Crisis Has No Easy Answer

Late in the game you're asked to help the Hubologists get fuel for their shuttle and separately help the Shi get fuel for an old oil tanker, but for a long time there was effectively only one source of fuel to go around. Players have spent years arguing over the exact order of quests needed to satisfy both factions without it quietly breaking one of the game's endings.
