South of the Derelict Manor waypoint in Ashwold Cemetery, there's a cracked stone wall that looks like nothing until you get close and see a prompt to 'Enter Hidden Crypt.' It's not tied to any quest, it's just a fully unmarked mini-dungeon with elite enemies guarding a loot chest for anyone curious enough to poke at the scenery.
